Bluegum Ability Exit and Transition Policy

Bluegum Ability recognises that exiting and transitioning to a new service can be a daunting and stressful time for the individuals we support and their family members. We are committed to providing professional and well-planned support throughout this process. This procedure applies to individuals who choose to commence receiving support from another service provider.

NDIS Obligations

As a provider of NDIS-funded Disability Supports, Bluegum Ability is obligated to ensure:

Transition for Clients Exiting Bluegum Ability Services

When a person we support is considering exiting Bluegum Ability services or transitioning to another provider, our staff will meet with the individual and their support network to discuss their reasons for leaving. We aim to understand any issues we may assist with or make adjustments to improve their experience.

Bluegum Ability staff can assist the person in exploring whether:

During this process, the person should be supported in a manner that allows them to exercise their right to have choice and control over their supports and services, as well as how these are delivered. If the individual, with their support network, decides to exit Bluegum Ability, we will support them in exploring alternative services as appropriate.

Additional Support Requirements and Modifications

All relevant information should be provided promptly to the new provider to ensure arrangements are in place before the person begins services. This includes any necessary modifications to a bedroom, access, vehicle, arrival of equipment, safeguarding measures, health requirements, behaviour support needs, and staff training.

Introduction to the Local Community

Introducing the person to their new environment, including housemates, staff, other clients, and the broader community, is an important part of the transition process. This may require several visits, depending on the person’s needs and available support. Bluegum Ability will work collaboratively with the new provider to ensure these introductions occur.

Maintaining Relationships, Friendships, and Community Connections

Where possible and appropriate, efforts should be made to farewell other Bluegum Ability clients, staff, service providers (if relevant to service delivery), neighbours, and the community. Established friendships and connections should be supported to continue post-departure, including arranging email, virtual meetings, phone contact, and visits.

Change of Address

It is essential to inform relevant service providers of the individual’s new address. This includes notifying NDIS, Centrelink, health professionals, Behaviour Support professionals, banks, and other essential contacts as needed.

Sharing of Information

Where the individual consents, key support documents should be shared with the new service provider to ensure continuity of support. If consent is not provided, the individual should receive a copy of their key support documents.

Exiting the Individual from Bluegum Ability Operational Systems

Completing the operational exit process ensures that individuals are removed from Bluegum Ability’s systems, maintaining accurate data and avoiding charges for services no longer received. Bluegum Ability will manage personal information in line with our Privacy and Confidentiality Policy Guideline.
